Job Summary

The Care Management Health Educator II works under the guidance of a Manager or Director of the Department as required. The Educator is responsible for conducting functional and technical training for the Care Management Services teams (including Care Management, Care Coordination and Community Health Work teams) in alignment with departmental policies and procedures. The Educator coordinates and/or provides training with the onboarding of new hires. This position is responsible for ongoing training, education, and remediation for experienced staff. The position must be a subject matter expert in case management processes and standards of practice including, but not limited to community, professional, and industry standards. This position serves as the chief instructor and consultant for operational, reporting, and accrediting requirements for all lines of business. 

 

The Care Management Health Educator II supports the Care Management Services teams in ensuring quality and compliance with the regulations and requirements of state, federal, and accrediting agencies. This position conducts educational needs assessments across all lines of business and performs instructional analysis, develops timeline/ training tools / resources to ensure training materials are compliant with professional case management standards of practice, quality, evidenced based outcomes, regulations, and accrediting guidelines.  In addition, this position assists with knowledge transfer, and “Train-the-Trainer” programs.

Duties

Partners with Leadership to develop and maintain project plans for developed courses, including new hire onboarding training for the Care Management Services department, developing training programs, preparing training schedules, establishing training/ educational goals/ objectives; and, conducting initial and ongoing evaluations of such. 

Responsible for developing staff training for technology/systems that support the work of Care Management Services staff.  

 

Applies alternative training mediums and methods (self-study, online, blended, simulations) to increase training effectiveness and to accommodate distance learning, learning style and cultural differences. 

 

Writes behavioral objectives, tests, training methodology, and course materials using instructional design theory and modalities. Manages databases and instructional hardware/software for documentation, testing, and maintaining all training materials including eLearning modules, scenarios, job aids and course materials. 

 

Collaborates with and assists Health Services Educators across the various Health Services departments as needed. Partners with the Health Services Provider Support Services Department in the development and implementation of cross functional trainings.  Works closely with Learning Technologies Administrator and provides backup as needed.  

 

Provides monthly progress reports and recommendations on learning plans to the Director of Care Management who determines the content and frequency of such reports.

 

Coordinates annual presentations, internal, and external educational opportunities. Provides oversight for all new CCMC certification applicants. Tracks and documents the progress of all staff matriculating in higher education programs.

 

Responsible for keeping abreast of updates in community, professional and industry practice standards. Serves as a Lead Preceptor/Trainer to effectively deploy policy/procedure updates and regulatory requirements in a consistent and compliant manner.  

 

Actively contributes in updates to and maintenance of organizational policies and procedures as it pertains to Care Management in collaboration with the CM leadership team. 

 

Regularly performs quality check updates in job aides, and other materials to ensure regulatory compliance and implementation. 

 

Applies audit data to the development and improvement of training programs.  

 

Participates in reviews and preparation for internal and regulatory audits of Case Management records; analyzing audit findings; making recommendations for corrective action plans;  and implementing internal and external corrective action plans as it pertains to education and training of staff under the guidance of the Care Management Services leadership team.

 

Collaborates with leadership to develop a standardized process for team member audits and rebuttal as well as an audit tool.   

 

Develop a competency based orientation for all Care Management Services job descriptions.  

 

Partners with other Subject Matter Experts within LA Care to coordinate department educational opportunities.

 

Collaborates with internal and external partners to plan/ facilitate/ execute the educational and training goals of the department and organization including, but not limited to, the Provider Support Services Department and Human Resources.

Education Required

Master's degree in Social Work OR Associates Degree in Nursing
In lieu of degree, equivalent education and/or experience may be considered.

Education Preferred

Bachelor's Degree in Nursing or Related Field

Experience

Required:

A minimum of 5 years of experience in designing, developing, and delivering in-person and online training courses; or any combination of education and experience which would provide the required knowledge, skills, and abilities.

 

At least 3 years of progressively responsible work experience in Care Management (in an acute or managed care setting, and/or in training and education).

 

Knowledge and experience with eLearning tools and internet technology.

 

Preferred:

Experience working in a health care setting providing individual case management/counseling strongly preferred.

Skills

Required:
Knowledge of healthcare industry trends, directions, major issues, regulatory considerations and trendsetters.

 

Knowledge and understanding of legislation and regulatory bodies affecting healthcare practices.

 

Knowledge and understanding of the Health Insurance Portability and Accountability Act.

 

Knowledge of and ability to adopt various tools and methods to conduct pre- and post-training testing and associated analysis for test results with the purpose of improving training effectiveness.

 

Knowledge of and ability to identify what people need to learn for successful individual and organizational performance.

 

Knowledge of and ability to administer procedures, tools and logistics for a variety of training programs.

 

In-depth proficiency in MS PowerPoint, Articulate Storyline, SCORM, Adobe Captivate, or other rapid eLearning and video development software/tools. Proficient in Visio, Share Point, Word, Excel, and Outlook.

 

Ability to demonstrate critical thinking skills, high level of organizational and time management skills, strong analytical skills. 

 

Must demonstrate flexibility to meet changing training needs and must be a strong team player willing to work to accomplish team goals.

 

Able to demonstrate experience / knowledge in lesson and / or curriculum planning.

 

Outstanding, written, oral and presentation skills. Able to write effective and accurate instructional text and demonstrates excellent written and verbal presentation skills.

 

Skills in measurement and evaluation of training initiatives.

Preferred:
Knowledge and experience with Adobe Flash. Experience with Success Factors strongly preferred.

Licenses/Certifications Required

Registered Nurse (RN);current and unrestricted California License OR Licensed Clinical Social Worker; current and unrestricted California License.
