Job Summary

The Clinical Social Worker II provides a wide variety of professional social work services within the Social Services Department. This position is key in implementing and executing emerging initiatives in social services. This position ensures all special programs and initiatives functions are operating in accordance with the organization's mission, values, and strategic goals. This position represents L.A. Care and interacts with various stakeholders, community partners, and Community Based Organization (CBOs) in a manner that promotes collaborative working relationships. This position provides clinical consultation services to other L.A. Care staff and provides training on special initiatives/program within Social Services. The Clinical Social Worker II provides direct clinical work and maintains a caseload of complex cases.

Duties

Performs a wide variety of professional social work services within the department and organization to support special programs and initiatives within the department/organization. This includes special projects, including but are not limited to: Orientation and training of new social workers; Social Work Intern Programs; Homeless program initiatives; Special programs addressing social determinants of health and other cross-functional team programs developed in Health Services. Performs project based activities including program support to ensure successful development/implementation of existing and emerging initiatives.

Participates in professional committees, meetings and conferences, related to the special projects and initiatives in Social Services. 

Manages projects assigned to ensure compliance with program policy. Acts as Subject Matter Expert and provides leadership and guidance to represent special projects and initiatives within Social Services. This includes providing technical support and training related to emerging projects. Analyzes data and produces reports to the department and the leadership team to help manage the projects service delivery.    

Develops  and maintains effective working relationships with other services areas within the organization and external partners. Serves as a liaison between Social Services Department and external partners, including but not limited to working with CBOs, PPGs, hospitals, stakeholders, universities, and collaborating partners.  

Interviews individuals, their families, and/or caregivers to complete psychosocial assessment. Identifies and analyzes the member's needs for services. Sets priorities and treatment goals based on the member's needs. Develops plans to attain identified goals with the help of the member. Provides consultation and information to clients, stakeholders, internal and external departmental professionals, particularly for complex cases. Will maintain a caseload of complex cases.

Master's Degree in Social Work

Education Preferred

Experience

Required:

At least 3 years of experience working with adults, seniors, persons with disabilities, individuals with chronic health conditions, and/or dually diagnosed individuals in health care setting or social agency.

 

Preferred:

Managed care experience, preferably in a care/case management role.

 

Previous experience with a health plan.

Skills

Required:

Knowledge of managed care and an understanding of Medi-Cal and Medicare programs.

 

Must be highly motivated and a self-starter.

 

The ability to communicate with and relate to a diverse group of people including clients, community, and other staff.

 

Must have excellent administrative and organizational skills.

 

Strong customer services and excellent verbal and written communication skills.

 

Strong analytical, conflict resolutions and persuasion skills.

 

Proficiency with Microsoft Office including Outlook, Word, and Excel.

 

Ability to effectively negotiate with internal and external providers of patient care services.

 

Ability to problem solve in a proactive, creative manner, using sound judgment based on factual information and clinical knowledge.

 

Ability to develop leadership skills and to serve as a role model for clinical staff.

Preferred: 
Bilingual in one of LA Care Health Plan’s threshold languages is highly desirable. English, Spanish, Chinese, Armenian, Arabic, Farsi, Khmer, Korean, Russian, Tagalog, Vietnamese.

Licenses/Certifications Required

Licensed Clinical Social Worker (LCSW) - Active, current and unrestricted California License
