Job Summary
The Health Homes Coordinator II provides a broad range of project and operational support to the Health Homes team and reports to the Manager, Health Homes. This position will be responsible for coordinating and organizing Health Homes Program activities, including preparation of meeting materials and general administrative support to project leads with any necessary duties. This position supports the Health Homes program administration team in the monitoring and oversight process by collecting information, communicating with members, providers, and internal departments, and tracking program and compliance reports. This position also supports referrals to the Health Homes program by handling intake, data entry, processing, and status updates. In addition, this position maintains confidentiality when communicating member information, assists with the communication and coordination between programs, and performs other duties as assigned.
Duties
Provide organizational and administrative support for Health Homes Program activities to help achieve desired program goals.
Plan and coordinate meetings, events, and related project actives to facilitate the optimal performance of the Health Homes Program. Handles scheduling and coordination for stakeholder and provider meetings; correspondence, and follow up reports; compiles meeting materials; and maintains updated information on Program status. Assist with disseminating program communications to internal and external stakeholders.
Provides assistance to team members as needed to achieve department goals. (30%)
Supports the Health Homes program administration team in the monitoring and oversight process.
Maintains the regular reporting responsibilities for Health Homes; tracks report timeliness, and notifies team of delays. Assists with soliciting non-clinical information from providers concerning member cases and program operations, including follow up and clarifications. Provides non-clinical support to team members in the Health Risk Assessment and Health Action Plan review process (20%)
The Coordinator supports referrals to the Health Homes program by handling intake, data entry, processing, and status updates.
This includes the technical aspects of the time sensitive processes for initiating cases, managing referral documentation, referral to other related programs (if applicable), identifying and responding to urgency of the request, appropriate documentation, case routing and tracking, routing of information, performing computer data input, faxing, filing of confidential member information, creating letter templates, and maintaining logs of activity, etc. (40%)

Experience
Required:
Minimum of 0-1 years of coordinator experience in health care/health services, with at least 1 year of experience in Medi-Cal managed care and/or as a Medical Assistant.
Skills
Required:
Knowledge of medical terminology and ICD-10 and CPT codes.
Strong verbal and written communication skills.
Proficiency with Microsoft Word, and Excel.
Excellent organizational, interpersonal and time management skills.
Must be detail-oriented and a team player.
Persuasion Skills: Must be able to interface with members, medical personnel, and other internal and external agencies and sometimes convince/persuade others to comply with L. A. Care requirements and provide information in a timely fashion.
