Job Summary
The Housing and Homelessness Incentive Program System Liaison II will work with the Housing and Homelessness Incentive Program (HHIP) which is a voluntary Medi-Cal Managed Care Plan (MCP) Incentive Program that aims to improve health outcomes and access to whole person care services by addressing housing insecurity and instability as social determinants of health for the Medi-Cal population. This position meets the HHIP Measure 1.2 requirement by improving connections between L.A. Care and the Coordinated Entry System (CES). This position will be responsible for collaborating, supporting and pursuing connections between MCPs and CES lead agencies. This position would serve as a “bridge” between MCPs and CES systems / providers. In a close collaboration with the Los Angeles Homeless Services Authority (LAHSA) health care liaison and other local Continuums of Care (CoCs), the CES liaisons would be a resource to help CES staff/providers get members connected to the right resources at the MCP. This position will also be a resource to MCP staff to help them connect members with the right resources within the CES system. Ultimately, the CES Liaison will help L.A. Care and its CoC partners work together to achieve the triple aim of better health quality outcomes, better patient experience of care, and lower cost of care.
Duties
Assess homeless member’s needs and connect homeless members to receive services through the CES Hubs. Connect members to the appropriate CES Hub. Help the member navigate through this resource by providing information for access centers. 15%
Assess homeless member’s needs and identify interventions that best suit the member’s needs by helping connect them to the appropriate resources and programs. Provide Follow up with member to make sure connection was made. Collaborate with referring entity to offer a warm handoff process and ensure member engagement. 15%
Provide technical support, strategies and resources to address complex cases for un-housed and previously homeless members receiving services through CES system, interim housing setting and homeless outreach programs. Will assess complexity of cases and escalate cases within L.A. Care to receive the appropriate level of intervention. 10%
Provide training and support to external partners about L.A. Care resources and available programs. Collaborate with LA Care Community Supports business units and provide appropriate guidance to match member with best resource. Monitor complex cases to ensure member receives services through the appropriately matched Community Supports Program. 10%
Build relationships and partnerships with CES staff at the COCs and with CES lead agency providers. Educate CES staff and CES lead agency providers on what the health plan offers (benefits, transportation, CS, ECM). Assist CES staff with support to navigate through L.A. Care internal programs and benefits. 10%
Fulfill role as liaison between LA Care and CES. Report back viable information to continue to enhance collaboration that will enhance workflow and processes. Share information back to the rest of the Health plan about challenges facing homeless members and homeless providers. Identify new problems that need solutions 10%
Duties Continued
Educate internal MCP staff re: CES processes and how to work effectively with members experiencing homelessness & non-contracted homeless services / housing providers. 5%
Relationship building: maintain positive working relationships with internal L.A. Care management staff, and external business partners in order to facilitate mutual collaboration in program planning efforts. 5%
Representation at internal & external initiatives: with guidance from an experienced staff member or manager: Represent the Team in external stakeholder meetings and participate as directed by Manager. Represent department at L.A. Care committee standing meetings and management meetings. Provide input regarding progress of projects to external and internal business partners. 5%
Educational Presentations: with guidance from an experienced staff member or manager, develop and deliver educational presentations in small and large group settings on programmatic research and findings. 5%

Experience
Required:
At least 0-2 years work experience in health care, homeless services, or a related field with the demonstrated experience on the following list of stated knowledge/experience.
Experience working with people experiencing homelessness and with community-based providers.
Preferred:
Experience helping providers and members navigate public programs.
Skills
Required:
Knowledge of Medi-Cal Managed Care benefits and programs; knowledge of Los Angeles homeless services; familiarity with the Los Angeles Safety Net;
Skilled at navigating within the health plan / health care system and the homeless services system.
Strong problem solving skills and follow through to ensure case closure.
Strong communication skills for both member and provider conversations.
Ability to juggle multiple urgent cases and seek quick resolution.
